Honor My Decisions for iPhone app for iPhone and iPad

You must be an Honor My Decisions subscriber to use this app (you may subscribe via our iPad app). With this version you will be able to create and edit your advance care planning documents but not create a new account.

Make sure your medical wishes are followed when you cant speak for yourself with a comprehensive and accessible advance care plan. Create or upload a written Living Will, record a Video Living Will, name Surrogates for healthcare decisions, list Emergency Contacts, upload a Last Will and Testament, and more with Honor My Decisions.

Your medical wishes are securely stored in the cloud, but can be easily retrieved when needed by emergency caregivers by scanning a QR code on your personal ID card (mailed to you after registration). No more lost or misplaced documents that can cause undue suffering, family strife, and financial burdens.

The Living Will and Surrogate forms are US state-specific (we cover all 50 states and 5 US Territories), so you can create an effective advance directive that will be recognized and honored in your community.
